NURA SA SERVICE PROVISION AND HEALTH INFORMATION SHARING

Client Consent and Authorization:

You (the customer) grant Nura SA the following consents and authorizations:

a. Access to Doctors and Health Practitioners:
– You authorize Nura SA to access your doctors, healthcare providers, and other relevant
practitioners and service providers involved in your health journey, for the purpose of
coordinating and delivering comprehensive Nura screening services. This includes obtaining
medical records, test results, and other pertinent health information.

b. Sharing of Health Information:
– You acknowledge and agree that your health information, including medical records, test
results, treatment plans, and other related data, may be shared among the healthcare
practitioners, research partners, and service providers involved in your Nura health journey.
You acknowledge and consent to your information being used solely for the purpose of providing
the agreed-upon health and Nura screening services and coordinating your health and Nura
journey with Nura SA.

c. Confidentiality and Privacy:
– You acknowledge and understand that Nura SA and the practitioners involved in your health and
Nura screening services journey shall maintain strict confidentiality of your health information
in accordance with applicable privacy laws and regulations and access to health information will
be restricted to authorized personnel involved in the provision of services or as required by law.
– You understand and accept that electronic communication, such as email and online platforms,
may be used for sharing health information.
– You acknowledge and understand that while reasonable measures will be taken to secure
electronic communications that no method of transmission over the internet is entirely secure.

d. Termination:
– You acknowledge and understand that either party may terminate this Agreement upon written
notice to the other party.
– You acknowledge and understand that termination does not relieve either party of any
obligations incurred prior to the termination date.

e. Governing Law:
– You acknowledge, understand, and consent to your relationship with Nura SA being governed
by and construed in accordance with the laws of The Republic of South Africa.
You acknowledge, understand, and consent to any disputes arising out of or relating to this
Agreement being subject to the exclusive jurisdiction of the courts in The Republic of South Africa.
